This exam verification form is for an employer seeking exam verification information on an O.D. for employment. NBEO^® develops, administers, and scores the entry-into-the-profession examinations that are used by regulatory boards of optometry in making licensure decisions. In order to request exam verification information, the employer must also submit the NBEO Release Form signed by the candidate.

NBEO does NOT maintain information regarding the licensure status of optometrists.

NBEO will verify the following:

• The examinations taken
• The month & year the examinations were taken
• The pass or fail status ("P" or "F")

Exam Verification Fee: $60.00

The verification will be electronic to the email address used at the time of purchase and may take up to two business days, once NBEO recieves a complete request form.
