NATIONAL BOARD EXAMINATION CREDENTIALING ONLY

This exam verification form is for a third-party (i.e. an employer) seeking exam verification information on an optometrist. NBEO^® develops, administers, and scores the entry-into-the-profession examinations that are used by regulatory boards of optometry in making licensure decisions. In order to request exam verification information, the third-party must also submit the NBEO Release Form signed by the candidate.

NBEO does NOT maintain information regarding the licensure status of optometrists.

NBEO will verify the following:

• The examinations taken
• The month & year the examinations were taken
• The pass or fail status ("P" or "F")

Exam Verification Fee: $60.00

The verification will be electronic to the email address used at the time of purchase and may take up to two business days, once NBEO recieves a complete request form.
