Part III CSE®, ISE and LSPE

To be admitted to the test center, Candidates must present an acceptable form of picture identification that includes both a photograph and a signature. The only acceptable forms of identification are:
  • A valid driver's license or an official photo ID issued by the government of the state or province where the Candidate resides.
  • A valid passport.
  • A valid student identification card from an accredited school or college of optometry, provided this ID card includes both a photograph and a signature embedded in the card.
  • In order to be considered valid, the ID must match the name on the roster for the exam. If the name does not match, a Candidate may be denied access to the exam. ID's from membership organizations are not permissible for admission to any National Board test.

During admission to the test center, Candidates will be required to sign their name as further proof of identification. This signature becomes evidence for any future challenge regarding the identity of the test taker.

Computer-Based Testing (Part I ABS®, Part II PAM®, TMOD®, CPDO®, ACMO®)

You will be required to show two (2) valid forms of personal identification. In order to be considered valid, the ID must match the name used to register for the exam. Both forms must contain your signature and must be current (not expired). At least one form must be a government-issued photo ID, such as a driver's license, a passport, or a military ID. The other ID may be a student identification card, a credit or debit card, or other card that has your name and signature. Candidates who do not have proper ID may be denied admission to the exam and are not eligible for a refund of the exam fee.
